This form is for initial intake and firm's evaluation purposes only. Accordingly, any consultation does not create a formal attorney-client relationship upon which you can rely. You acknowledge that there is no retention until (1) execution of a legal services agreement and (2) payment of an initial retainer. You are further advised that Firm is not responsible in the event you later fail to timely take action on your matter, and that you are responsible for all deadlines to file any complaint or other legal process. Accordingly, Firm urges you to be aware of any such deadlines in your matter to avoid waiver of any or all of your rights.

Further, any documents you may provide to firm are copies that firm may destroy at its discretion at any time, and you therefore confirm that you are not providing any originals or other documents to Firm that cannot easily be replaced. You understand that Firm will not receive any such irreplaceable documents until and only if there is an actual retention of firm pursuant to a separate attorney legal services agreement which we both enter into.
